Children’s Long-Term Support (CLTS)

The Children’s Long-Term Support (CLTS) Program offers supports and services for children and young adults under age 22 who have significant developmental, physical, or emotional disabilities and substantial limitations in multiple daily activities because of these disabilities.

The CLTS Program helps children and young adults with disabilities live at home and better participate in their family and community life. It includes services and supports that help participants:

  • Complete daily living tasks.
  • Enhance relationships with family, friends, and community members.
  • Learn new skills and develop new interests.
  • Achieve their best physical and mental health.
  • Keep living at home.
  •  Live their best life.

 For those that are already enrolled, the CLTS Participant and Family Handbook can help navigate the program and understand their rights.

DETERMINING ELIGIBILITY

A functional screen is completed to determine eligibility.  Youth may qualify under three different target groups: physical disability, developmental disability, and mental health. Youth must also have a source of Medicaid to qualify but this can be provided through CLTS if needed. Parents may have a cost share based on their income, family size, and cost of services.

MAKING A REFERRAL

Referrals can be made to the Family Navigation Specialist at the Parenting Place.  Click on the link for Children with Special Needs Programs - The Parenting Place, scroll down to “Family Navigation Services” and click on the “referral form” button.
